Fellowship Payments to Foreign Students with F-1 or J-1 Visas

This is a reminder for departments issuing fellowship stipends - financial support beyond tuition and fees - to international students with F-1 or
J-1
visas.

Such stipends cannot be paid until the student submits the following required documents for the calendar year 2007 to the Disbursements Office:

1) U.S. Taxpayer ID Number: SSN or ITIN*

2) A completed UC W-8BEN (available at
http://www.ucop.edu/ucophome/cao/paycoord/w8ben.pdf)

3) Copy of I-94

4) Copy of I-20 (F-1 visa holder) or DS-2019 (J-1 visa holder)

5) A completed tax treaty statement for scholarship and fellowship grants (available at http://www.ucop.edu/ucophome/cao/paycoord/taxstate.html)

Please ask your students to submit the required documentation as soon as possible, if they have not already done so, to Christine Chang, Disbursements Office, 2195 Hearst Avenue, Room 125, MC1101. Once their documents are received, the fellowship funds will be released to the student.
